Instructional CoursesThe fee for each course is US$30 Optional Gala Dinner at the CN Tower, TorontoAn optional gala dinner, at the CN Tower, Toronto, will take place 20:00 - 23:00 on Friday July 10, 2009. Unfortunately, places at the gala dinner are now sold out. PaymentRegistration fees are payable in advance of the conference. In order to expedite the registration process, please complete the registration form with your credit card details. Confirmation of your credit card payment along with registration confirmation documents will be sent to you by (air)mail. If you do not receive confirmation of your registration 7 days prior to the event, please contact the Conference Secretariat. Alternative payment methods: If you do not wish to pay by credit card please download the printable registration form and return to the conference secretariat. (The link opens a pdf in a new window. Alternative payment methods: Checks should be made payable to Elsevier and should be sent with your completed registration form. In order to pay by bank transfer, you will first require an invoice. An invoice will be sent to you by airmail upon receipt of a signed registration form completed with your contact details and registration details. The reverse side of the invoice provides Elsevier bank account details for transfer purposes. Please do not transfer fees until you have received your invoice. It is essential that you include your invoice number when instructing your bank. Please forward a copy of the bank transfer documentation to the Conference Secretariat. CancellationsSubstitutions may be made at any time but please advise the Conference Secretariat of a change of name. If you find it necessary to cancel the registration completely, please notify the Conference Secretariat immediately. Provided written notice is received by 27 May 2009, a full refund will be given less a 10% administration charge. Provided written notice is received 10 June 2009, a 50% refund will be given. It is regretted for registrations cancelled after 10 June 2009, or for no-shows at the conference, the full fee (100% cancellation charges) will be payable. In the unlikely event that Elsevier shall deem it necessary to cancel the conference, all pre-paid registration fees will be reimbursed. Elsevier shall not be liable for reimbursing the cost of travel or accommodation arrangements made by individual delegates. ChangesDetails are correct at time of going to print. The organizers reserve the right to change any session in the final program. PhotographyPlease be advised that no photography or video/sound recording of conference presentations is allowed to take place during the conference. |
